>> Your Disclosures to the Fund
Everyone who is entitled to claim benefits from the Plan must furnish
to the Fund all necessary information in writing as may be reasonably
requested for the purpose of establishing, maintaining and administering
the Plan. Failure to comply with such requests promptly and in good faith
will be sufficient grounds for delaying or denying payment of benefits. The
Board will be the sole judge of the standard of proof required in any case,
and may periodically adopt such formulas, methods and procedures as the
Board considers advisable.
The information you give to the Fund, including statements
concerning your age and marital status, affects the determination of
your benefits. If any of the information you provide is false, you may
be required to indemnify and repay the Fund for any losses or damages
caused by your false statements. In addition, if a claim has been submitted
for payment or paid by the Fund as a result of false statements, the Fund
may seek reimbursement and may elect to pursue the matter by pressing
criminal charges. Knowingly claiming benefits for someone who is not
eligible is considered fraud and could subject you to criminal prosecution.
