This is the Summary Plan Description (“SPD”) of the plan of benefits (“the Plan”) of the Service Employees 32BJ North Health Fund (“the Fund”) with regard to the Tri-State Preferred North Plan. Your rights to benefits can only be determined by this SPD, as interpreted by official action of the Board of Trustees (“the Board”). You should refer to this booklet when you need information about your Plan benefits. In addition, the Board reserves the right, in its sole and absolute discretion, to amend the Plan at any time.

Your health benefits are determined by the collective bargaining agreement or contract negotiated between your union, SEIU Local 32BJ, and your employer. There are several health funds that cover members of SEIU Local 32BJ and each health fund may have more than one plan of benefits, depending on how much your employer is required to contribute.
The Tri-State Preferred North Plan covers members working in:
- Bronx Residential (except Co-op City)
- Boiler Repair
- Westchester Country Clubs
